Pharmacy Appreciation at BCHS

March is Pharmacy Appreciation Month and we would like to thank our remarkable BCHS Pharmacy Team for their continued dedication in providing vital services to our staff, along with exceptional care to our patients and community.

We would also like to introduce you to Fred Katbi, a Registered Pharmacist here at the Brant Community Healthcare System (BCHS). We sat down with Fred as he reflected on the essential role Pharmacy plays in our hospitals.

What role does the BCHS Pharmacy Team play?

Pharmacy plays two important roles at BCHS: a distribution and clinical role. When it comes to medication distribution, our hard working Pharmacy Technicians go above and beyond to ensure that medication orders are being processed in a timely manner, accurately dispensed, and stocked in their appropriate wards. Pharmacists fulfill the clinical role through order verification, answering drug information questions and collaborating with the interdisciplinary team.

Why is Pharmacy important?

From the patient side, pharmacists play a role in maintaining continuity of care for our patients through their transition in the hospital. Additionally, pharmacists serve as liaisons to patients by being part of their discharge planning and communicating with community pharmacies regarding patients’ medication regimen. Collaboratively, pharmacists work with other healthcare providers to optimize patients’ medications, while also ensuring safety and effectiveness.

How does Pharmacy positively impact the patient care plan and healing journey?

One major impact that I believe Pharmacists have on patient care plans is reducing polypharmacy and prescribing cascades. With that in mind, Pharmacists constantly review patients’ medication regimens and aim at simplifying them.

The complexities surrounding medication management may be overwhelming, and as such, Pharmacists provide accessible and sustainable means to help patients navigate their recovery journey.
