What is the Brantford Brant Norfolk Ontario Health Team?

Ontario Health Teams (OHT) are being introduced across the province as a new way of structuring and delivering care that is more connected to patients in their designated community. Under OHTs, health care providers (including hospitals, doctors, and home and community care providers) work as one coordinated team.

The Brantford Brant Norfolk Ontario Health Team (BBNOHT) works to provide information, support and access to health services in the Brantford-Brant community. It's vision is to have multiple organizations working together through an integrated care model; with a focus on communication and collaboration to maximize outcomes for more patient-centered care. 

Brant Community Healthcare System is proud to be one of the 27 signatory agencies spanning the health care and community service continuum in Brantford, Brant County and Norfolk.

What This Means For You 

You do not need to change the way you get care. Still go to your primary care provider if you are feeling sick, your counsellor if you are feeling unwell, or the hospital if there is an emergency. If you currently have home care, you will continue to receive home care. 

The BIG CHANGE is that multiple organizations will work better together. When you need care, it will feel more like you are being cared for by one organization rather than being "discharged" from one service into another.

Building a Foundation for a Healthier Community

The BBNOHT’s strategy is grounded in population health with the aim to improve health outcomes and patient experience for the priority focus areas of addictions and mental health, dementia care and homelessness in Year 1.

Additionally, the BBNOHT is embarking on a digital health strategy to facilitate care coordination and information transfer. Strong connections and partnerships with the Indigenous population at the Six Nations of the Grand River, Mississaugas of the Credit First Nation, and the urban Indigenous population is foundational work of the BBOHT.
