Patient Surveys

If you've received hospital care recently at BCHS, you may be contacted by Qualtrics - a third party firm working with BCHS to survey our patients and their families. 

Brant Community Healthcare System is focused on working together to build a healthier community. Our main priority is to deliver safe, high-quality care and exceptional patient experience to every person, every day.

Learning what patients think and feel is crucial to providing the best possible care and improving the patient experience. A patient’s viewpoint helps providers see the complete picture of the healthcare quality and experience, checking in to see if the right standards were achieved.

Patient surveys are surveys that ask specific questions about the patient’s experience. They are designed to create a crucial connection between healthcare staff and patients.

If you are contacted, please take a few minutes to answer the survey questions. We anticipate it should take no longer than ten minutes. We rely on you to let us know what we're doing right and where we must do better. Your input and ideas will help us improve the future care and services provided to our patients. 

Your Privacy

Participation in this survey is optional but greatly appreciated. Some of the questions on the survey will ask you about your ethnicity, gender identity, and other basic demographic information. Your responses to these questions help BCHS better understand the communities we serve and cannot be used to identify you. Your participation in the demographic questions is optional, even if you wish to complete the rest of the survey.
