How to Request Personal Health Records
We respect your right to access your own personal health information regarding the treatment you received at Brant Community Health Care System. We share your personal health information with health care providers so they may treat and assess you. To access your information you may speak to your physician or contact our Release of Information office.

How do I get a copy of my personal health records? |

You can receive a copy of your health records by providing a signed consent form, prepayment, photo identification and legal documentation if necessary. |

What happens if I am inquiring about records for a deceased patient? |

To obtain records for individuals who are deceased, original legal documentation of a will is required. If these documents are not available please contact Release of Information staff for further instructions. | ||||||||||||||
Does it cost me anything to request medical information? |

To cover the cost of time and supplies there is an administrative fee for requests. The base fee structure is set by the Information and Privacy Commissioner of Ontario. Additional fees may be charged by BCHS according to the table below:

What is a patient portal? A patient portal is a secure online website or app that gives you convenient, 24-hour access to your personal health information from anywhere with an Internet connection. Using a secure username and password, you can view your hospital health information electronically from participating organizations/sites across South Western Ontario. MyChart to be replaced with ConnectMyHealth Patients who have a South Western Ontario (SWO) MyChart account can continue to have access to their data from participating regional hospitals via MyChart until March 31, 2023 (extended from the previously-communicated December 31, 2022). If you already have a MyChart account, please be advised that after March 31, 2023, you will not be able to use your MyChart account to view your health records from hospitals located in South Western Ontario. A new patient portal called ConnectMyHealth will soon be available for registration, that will allow you to view your health records from hospitals located in South Western Ontario. Important: Patients who did not already have a SWO MyChart account prior to the published September 14, 2022 registration cut-off date will not be able to create/register a new SWO MyChart account. However, they can continue to access their health records from the health care facility where they have received care. |
