Leaving the Hospital
At Brant Community Healthcare System, we believe recovery begins at home. The Home First philosophy is all about supporting patients—especially seniors—so they can safely return to a familiar environment with the right care in place.
After a hospital stay, many patients will continue their recovery safely in the comfort of their home with the right supports. Your healthcare team will work with you and your loved ones to determine the most appropriate resources and supports available for you.
With Home First, discharge planning starts early, and home is always the first option.
Here’s how Home First benefits you:

- Connects you with community-based supports
- Reduces exposure to hospital-based illnesses
Before You Leave the Hospital:
Take all personal belongings home (including your health card and any home medications you may have brought with you).
Ask about:
- After-care instructions
- Prescriptions
- Follow up appointments
- Any special equipment you might need
Your care team will work closely with you to create a plan for when you leave the hospital. They may also work with local organizations (like Ontario Health atHome) if needed. You and your caregivers are an important part of making these plans.
