Available in 2.5 gm, 5 gm, 10 gm, 20 gm, 30 gm or 40 gm vials depending on the manufacturer.

Physician MUST order total grams required.

Physician must complete the appropriate IVIG request form (see links above) and fax/send request form to the Laboratory for approval.

• when the dose is modified
• when six months have elapsed since the initial treatment
• when twelve months have elapsed for renewal requests

Order Enter the product request in Meditech.

Transfusion Services Technologist will confirm dosage, assign and issue the product.
