The Unit where death occurred is responsible to order enter the autopsy request in Meditech. A notification for requested autopsy automatically prints to Switchboard. Once notification is received at Switchboard, Switchboard email Notification to UHN via email to Michael.Seidman@uhn.ca and PathologistsAssistants@uhn.ca The UHN Requisition and Consent (available on the UHN Website) will be included in the email Switchboard then arranges transport to UHN .
