Collection Information

I.1. All specimens must be accompanied by an Ontario Health HPV and Cytology Testing  Requisition. Choose the appropriate req for screening or for Colposcopy. For patients who do not have a valid health card number, use the Dynacare Cytology Non-OCSP-OHIP Requisition. Document the account number on the requisition.  The Non OCSP requisition can also be used for IFH (such as refugee) patients. Write IFH in the client field and include the patient's IFH number in the HCN section or in the remarks section. 

 

The following information is required on the requisition:
(a) Patient identifiers (addressographs may be used)

(b) patient's first and last name
(c) date of birth
(d) Patients contact information
(e) Ontario Health Insurance Number with version code.
(f) Submitting area identifiers
(g) Primary care providers name and other physician's names to receive a copy of the report
(h) Requester or Colposcopist information and contact information

(i) Clinical/Testing Indication

(j) Last Menstrual Period (first day)

(k) Date of Specimen Collection

(l) Specimen information
(m) specimen site/type/source
(n) Special considerations for cytology interpretation

1.2 Label the Hologic Thinprep Preservation fluid collection vial with a minimum of two patient identifiers. Attach the meditech generated label. Do not cover the barcodes on the vial.


I.3. Place specimen container in a biohazard bag. The completed requisition should be placed in the outer pouch of the biohazard bag along with any additional meditech labels.

 

I.4. Transport specimen to the Triage area of the laboratory as soon as possible
