Where is it located?

The Transitional Care Unit is located on the second floor of the Willett Urgent Care Centre. Please refer to the maps below for the location. 

What is a Transitional Care Unit

The Transitional Care Unit (TCU) at the Willett Urgent Care Centre is part of the Brant Community Healthcare System (BCHS). The TCU supports patients who have completed their acute care stay but may benefit from additional support and care coordination while their next stage of care is being finalized.

The unit helps bridge the time between hospital care and the next step in a patient’s care journey. During this period, patients receive care in a setting focused on preparing for a safe, timely transition from the hospital.

This approach reflects Ontario’s Home First philosophy, which prioritizes helping people return home whenever it is safe and appropriate, with services in place to support that transition. When returning home is not the best option, the care team works closely with patients and families to plan for the most suitable next destination in the community.

At BCHS, this work is supported through partnerships with BCHS@Home, Ontario Health atHome, and various community support services that help connect patients with the care and services they may need after leaving the hospital.

Who the Unit Serves

The TCU is designed for patients who:

  • are medically stable and no longer require acute hospital treatment.
  • would benefit from additional support while plans for their next stage of care are being finalized.

What Patients Can Expect

Patients in the TCU receive care from an interdisciplinary team focused on preparing them for their transition from the hospital.

Planning for the next stage of care begins while patients are still in acute care. The TCU team builds on the work already underway, collaborating with patients and families to review options, coordinate services, and prepare for what comes next.

Patients and families are important partners in this process. The team provides information, recommendations, and support to help guide decision-making, while patients and families share their preferences and participate in planning for the next step. Staying engaged in discussions and responding to planning decisions in a timely way helps the team coordinate services and support a smooth transition from the hospital.
