Community Well-Being Teams

Rationale. The Brant Community Healthcare System recognizes the value of community input in the development, implementation and monitoring of wellness programs. The Community Well-Being Team is an avenue for that input. CWBT provides opportunities for the development, implementation and evaluation of health promotion and illness prevention strategies through community involvement and partnerships.

Areas Served. Paris and surrounding communities (eg. Brantford, St. George, Ayr, Princeton, Burford, Onandaga, Oakland/Scotland).

History. The CWBT evolved from a community program advisory committee into its present format as a committee that plans and implements its own programs.

Health Promotion Strategies. Health education and health communication are commonly used strategies.

Activities. In collaboration with community and government agencies workshops, seminars, clinics are offered to the community on various health topics. Examples include: anxiety seminars, gardening workshops, cooking demonstrations, seniors tax preparation clinics, teddy bear clinics, mental health seminars, Tai Chi, Health Fairs, panel presentation on caregiving issues, memory loss, etc. Most programs are provided free of charge.

Target Population. The target populations include adults, infants and children. Most programs have a specific target audience (eg. women concerned about heart disease, children aged 8 and under for Teddy Bear clinic, women caring for an elderly parent or spouse).Teams. Community based, volunteer driven well-being teams exist in:

  1. Paris, St. George, Princeton, Burford, Oakland/Scotland, Onandaga

Outcomes:

Short-term.

  • Reinforcement of healthy lifestyle practices
  • Increased awareness of local services and supportsIncreased access to service

    Long-term.
  • Increased number of people adopting healthy lifestyle practices and experiencing improved health.

Resources Required. A member of the Community Health Services department provides administrative and logistical support to each team. Each program has a team leader and 3 – 4 volunteers that help implement the program. Activities often take place at The Willett, Paris but other community facilities are utilized (eg. library, Legion, Churches, grocery store). Supplies, refreshments, speaker honorariums and advertising are required for most programs. Donations to CWBT are accepted to help offset minor costs.
