How to book an appointment:

Once you have a referral from your physician, call:

751-5523 to book an appointment at Brantford General Rehab.

751-5506 to book an appointment at the Brantford Rehab Centre. Please have your extended health insurance information available to give to the secretary.

What to bring with you:

Ontario Health Insurance Card

Brantford General Hospital "Blue Card". If you don’t have a current card, please obtain from the Admitting department on the first floor of the hospital prior to your rehabilitation visit.

Appropriate clothing depending on the reason for your visit ie shorts, tank top, etc.. This allows the physiotherapist or occupational therapist to conduct a thorough assessment.

The referral slip from your physician if you have one.

What happens at your first visit:

Please plan to arrive a few minutes before your scheduled appointment time to complete any necessary paperwork.

During your initial visit, the physiotherapist or occupational therapist will gather medical information and perform physical assessment procedures specific to your condition, in order to formulate a physical diagnosis and develop a treatment plan.

Further appointments will be scheduled based on the recommendation of the therapist.
