THE COMMUNITY REHABILITATION PROGRAM

The Community Rehabilitation Program, located at the Brant Community Healthcare System, offers a range of medical, nursing and rehabilitative services to individuals who don't need 24-hour hospital care but would improve with involvement in a hospital-based therapeutic program.

The Program

The Community Rehabilitation Program provides specific outpatient rehabilitative and health care services, and promotes early discharge from acute and rehabilitation hospital beds. It is an alternative to hospitalization or institutionalization for patients who are well enough to go home but would benefit from further rehabilitation. The patient population served present primarily with neurological conditions, amputations and complex musculoskeletal conditions and require multidisciplinary care.

  • Occupational Therapy
  • Physiotherapy
  • Nursing: Health and Wellness Promotion
  • Speech-Language Pathology
  • Pain Self-Management Program
  • Amputee Program; Prosthetics and Orthotics
  • Stroke and Neurological Rehabilitation Program
  • Geriatric Wellness Program

Patients must:

  • Need two or more Community Rehabilitation Program services
  • Possess the potential to participate in a rehabilitation program that is goal-oriented and time-limited
  • Be willing and physically able to attend and be actively involved in the program

Referrals

Any hospital, health care agency or physician may refer patients to the Community Rehabilitation Program. A physician must sign completed referral forms before patients can be accepted into the program. The intake process includes an initial intake assessment to determine eligibility for the program and service needs.

Attendance

Patients usually attend the program two or three days each week, depending on individual needs. Patients may be discharged if they are non-compliant with scheduled appointments.

Discharge

Each Community Rehabilitation Program patient will be reassessed at frequent intervals to prepare for eventual discharge when the maximum level of independence has been achieved. Appropriate linkages may be made with community programs for continuity of care after discharge. Follow-up is provided regularly when appropriate.

Patients who are unwilling or unable to participate fully in the program will be discharged.

Transportation

Transportation to and from the Community Rehabilitation Program remains the responsibility of the patient and family. Options available include Operation Lift, family members, city bus and taxis.

The following departments maintain a continuum of care for our Community Rehabilitation Program patients: Inpatient Rehabilitation Program, Chiropody Laboratory, , Pastoral Care, Cardiopulmonary Services, Radiology, Assistive Devices Program, Geriatric Assessment Clinic, Falls Clinic, Memory Clinic, Driving Assessment, and the Prosthetics and Orthotics Clinic.
