WHO THIS PROGRAM IS FOR

Persons with all types of heart disease
Persons who are at high risk of developing heart disease - high cholesterol, overweight, high blood pressure, diabetes

PURPOSE

The Brant Community Healthcare System has an active multidisciplinary approach to cardiac disease prevention and rehabilitation. The program has both inpatient and outpatient components, designed to assist you in returning to your optimal functional level - physically and mentally.

THE PROGRAM:

Medically and scientifically sound exercise program
Guidance and supervision through the exercise program
Family participation is encouraged in the educational component of the program
A referral from a physician is required

Internal Medicine Specialists
Family Physicians
Registered Nurses
Physiotherapists
Occupational Therapists
Kinesiologists
Registered Dietitians
Pharmacists
Physical Therapy Assistants

WHAT IS IN THE PROGRAM

Orientation Session
Educational Lectures
Stress Management and Relaxation Session
Exercise Sessions

  • an initial stress test is performed to assess your physical ability (given by a physician)
    a personalized exercise prescription is developed by the physiotherapist based on your stress test
  • the exercise sessions are 1½ hours in length and involve a multi-step work-out which includes stretching and strengthening exercises, treadmill, stationary bicycle, and step class; depending on your level of fitnessblood pressure and heart rate is monitored throughout the sessionexercise sessions are two times per weekyou are also expected to exercise on a regular basis at home - this would include a walking program or a stationary or regular cycling programyou are stress tested on a regular basis after entering the exercise program
  • attendance is recommended for one year
