Secondary Stroke Prevention Clinic
The Stroke Prevention Clinic is an outpatient clinic for people who have signs and symptoms of a recent stroke or transient ischemic attack (TIA).
The goals of the clinic are to reduce the incidence of stroke by:
Providing early assessments and appropriate interventions;
Providing rapid access to diagnostic tests; and
Providing education to patients and family members about risk factor management and stroke prevention.
How can I contact the Stroke Prevention Clinic? There are two stroke prevention clinics within Brant / Haldimand – Norfolk . They are located at:

How do I find out when my appointment is booked?
After you have been referred, you will be called to book an appointment day and time.
If you do not receive a phone call within four days, please call our booking clerk at (519) 442- 2251 ext. 6352
What services are available at the Stroke Prevention Clinic?
Rapid access to Stroke Physician and Stroke Nurse
Access to diagnostic procedures
Support and risk factor screening
Rapid feedback for referring physicians; and
Case management for:
Referral for neurological services if needed
Lifestyle and risk factor modification
Referral to Dietitian, Physiotherapist, Occupational Therapist, Speech Language Pathologist, Social Worker if needed
What happens after my first appointment?
The clinic will:
Send a report of your visit to your family doctor
Book any tests and give you a follow-up appointment if needed.
You will need to:
Call your family doctor and make an appointment for 2 weeks after you have been in the Stroke Prevention Clinic.
You may be given a binder containing information about how to decrease your risk of stroke/TIA. Bring the binder to your next visit with your family doctor to review with you.
You can go to the clinic if you have had a recent stroke or TIA. You need a referral by an emergency room doctor, your family doctor, or another medical specialist. Your family doctor is kept informed of your progress and treatment plans.
Important Information
The Stroke Prevention Clinic is an outpatient clinic and does not replace an emergency department.
If your signs and symptoms of stroke or TIA get worse from the time of your referral to the Stroke Prevention Clinic, call 911 and get to the nearest emergency department right away.
Recognize the warning signs of a stroke:
- Sudden weakness, numbness or tingling
- Sudden trouble speaking or understanding speech
- Sudden vision problems
- Sudden severe headache
- Sudden dizziness or loss of balance
If you think you or someone you know may be having a stroke, call 911 immediately. Even if these symptoms appear to go away quickly, seek medical attention immediately.
