Enhancing and supporting the care of the patient and family by linking available resources.
The Supportive Care Clinic offers a setting where a comprehensive history can be taken, current needs of the patient and family can be thoroughly reviewed and treatment can be planned, initiated and/or adjusted.
Who can be referred
Any palliative patient who is experiencing difficulty in symptom management or a request for guidance in managing symptoms..
How to make a Referral
A physician referral is required. Information needed includes - diagnosis - current medications - issues to be addressed - any other information you think pertinent.
Please specify: 1. Consultation only or 2. Consultation and initiation of treatment.
Consultation requests can be faxed to 751-5588.
Once Referred
*A referral generates an appointment.
Family and friends are welcome to accompany the patient.
A history and physical assessment is obtained from patient and family.
*A care plan is formulated.
*The consultation note is dictated by the clinic physician and sent back to the referring physican. (If a return phone call on the day of the appointment is requested please include where physicians can be reached.)
*If requesting initiation of treatment, prescriptions will be written, orders initiated and an explanation to patients and families given.
*Requests for support workers will be initiated i.e. social worker, C.C.A.C. spiritual support, pharmacy, Palliative Care Directory.
*Advanced Directives may be addressed.
After consultation, patient care and follow up returns to the referring physician.
