Patient Safety Coordinator
Full-time

The Patient Safety Coordinator will work in collaboration with all levels of leadership, appropriate committees and hospital staff within the organization to ensure that risk reduction, patient safety, quality improvement strategies and improvement recommendations are carried through. Reporting to the Manager, Patient Safety, Risk Management, Quality Improvement.

The Patient Safety Coordinator will participate in problem assessment, solution recommendation, implementation and follow up on corrective actions and the administration of the hospital-wide Patient Safety, Risk Management, Quality Improvement Programs to improve the quality of patient related and non-patient related services within the Hospital through integration with the hospital teams and/or services.

Serves as the point of contact for patient safety policies and activities and is a key person to help build and implement patient safety systems and promote the culture of safety for patients throughout the BCHS

PRIMARY RESPONSIBILITIES:

  • Co-ordinates and provides leadership in the hospital-wide efforts directed at the elimination of detected risks and the identification/prevention of potential risks
  • Provides direct support to the development, maintenance and operation of the patient safety program
  • Provides consultation and guidance to the Care & Support Teams in the formulation and initiation of safety plans, and risk reduction/prevention strategies
  • Review and monitor BCHS Pt Safety policies for adherence to and compliance with regulatory agency and accreditation standards requirements
  • Develops, implements and maintains a patient safety/risk management educational program. Develops and presents specialized in-services to Hospital staff, medical staff and all others deemed necessary to enhance awareness and disseminate information on patient safety, risk related and regulatory issues
  • Coordinates and ensures the investigation, review and disposition of reported events and quality review/root cause analysis processes, providing recommendations for future prevention strategies and facilitating implementation of all identified interventions
  • Provides support on quality and patient safety processes to assist section leadership and organizational committees with information to improve quality
  • Comply with all BCHS Risk Management, Patient Safety and Occupational Health & Safety programs, policies and processes to eliminate or minimize unsafe acts or conditions, including identification & reporting of near miss and actual events

QUALIFICATIONS:

  • A regulated healthcare professional with a Certificate of Competence from the respective College
  • A university degree in a clinical healthcare discipline, in conjunction with 3-5 years clinical acute care experience
  • Demonstrated knowledge of risk management, patient safety practices and quality management practices including risk assessment, continuous quality improvement principles and practices, performance metrics/measurement, statistical analysis/interpretation
  • Knowledge of Patient Safety principles (including root cause analysis and prospective analytical tools), performance improvement principles, statistical processes, including trend and comparative analysis 
  • Experience or familiarity with the CCHSA Accreditation and Required Organizational Practices for Patient Safety
  • Demonstrated ability to establish and maintain effective working relationships with a wide variety of staff at all levels while dealing with complex and controversial issues. Adaptability with high tolerance for ambiguous work situations, including the ability to negotiate and resolve conflict and mediate
  • Formal certification or a willingness to obtain certification in patient safety, risk management
  • Excellent interpersonal, diplomacy, communication (written and verbal), critical thinking, attention to detail and problem solving skills
  • Working knowledge of current legislation in health care (Public Hospitals Act, Mental Health Act, Quality of Care Information Protection Act, PIPEDA and the Coroners' Act) preferred
  • Computer Skills: proficiency in Microsoft Office Suites (including Excel) and other appropriate computer programs. Experience with RL Solutions Risk Monitor Pro an asset
  • Must be able to translate policies, theories and strategies regarding patient safety into action
