Cardiac Rehabilitation Program

The Cardiac Rehabilitation Program, Healthy Hearts, is a partnership between the BCHS & YMCA is designed for patients with heart disease. The purpose of the program is to assist you in returning to your optimal activity level both physically and mentally. You will participate in exercise classes to safely return to an active lifestyle to further promote ongoing cardiac and overall health.

The program uses a multidisciplinary approach to heart disease prevention and rehabilitation. The education sessions include lectures by a variety of healthcare professionals including:

  • Registered nurse
  • Physiotherapist
  • Pharmacist
  • Dietitian
  • Social worker 

The Cardiac Exercise Classes take place at the YMCA Brantford located at 143 Wellington Street, Brantford. You will be assisted through the process with help from a physiotherapist and a LiveWell specialist.

  1. Initial stress test ordered by your internist to assess your physical ability
  2. Initial assessment by a physiotherapist to determine your individual goals while in the exercise class
  3. Free continuing education regarding healthy lifestyle choices - STEP Up To A Healthier You
  4. Follow-up stress testing to monitor progress at the end of your program
  5. After participation in this program you will be encouraged to continue with your exercise program at home or in a community setting
  6. The cost of the program follows standard YMCA rates (monthly and yearly options available). As a charitable organization, the YMCA strives to serve all segments of the community, including those with limited financial means. Donors to the YMCA make this possible. If financial circumstances are limiting your ability to participate, assistance may be provided.
