Wellness & Healthcare Integration
Community Well-Being Teams
These teams play a key role in the work of Wellness & Healthcare Integration at the Brant Community Healthcare System.
Community Well-Being Teams are active in these areas (click on the links below to view events):
• Burford
• Oakland/Scotland
• Onondaga
• Paris
• Princeton
• St. George
The Well-Being Teams are made up of local volunteers and bring together people, ideas and resources to improve the health of communities.
They work with other community partners to improve rural access to health information and health services.
Programs and services are organized to improve the well-being of communities.
These teams began as a new seed sprouting in the rural communities serviced by The Brant Community Healthcare System (BCHS).

The projects were initially aimed at meeting the needs of women and seniors. They have now branched out to include younger children, youth/teens and parents.
By sharing ideas and information, the teams identify service gaps and work towards meeting these health needs in the community.
