Geriatric Assessment Program

This program is for older adults living in Brantford and Brant County, whether living independently or in a Hospital, Nursing Home or Retirement Home.

As people get older, they can experience weakness, memory problems or other symptoms. In older adults, these symptoms could be related to any number of health problems including depression, dementia or an underlying infection.

A Geriatric Assessment is a complete health examination which determines health problems so that we can best treat the symptoms. The Assessment looks at a person's physical, social, cognitive, emotional and environmental needs and abilities.

A Geriatric Assessment will identify:

Health problems.
How day to day activities are affected by these problems.
How to improve health, well-being and functioning.
   
Getting Started
Individuals are referred through their family doctor.
If you or someone you care for would benefit from this service, speak with your doctor.

Who does it

picture of geriatric assessmentA team of professionals, who specialize in the health problems of older adults, consults with the family, the physician and other health care providers. Suggestions for treatment and follow-up are sent to your family doctor.

Two appointments are usually needed for an assessment. The first is usually a home visit. The second, if needed, is usually in the outpatient geriatric clinic at The Willett Site, but arrangements for a second home visit can be made.
