Admitting
at the Brantford General site

This page contains information on services we have available for patients in our facility and information on the process of admitting.

Telephones

Individual telephones are available in each room on the nursing units (with the exception of MH3, MH4, ICU, Day Surgery and Ambulatory Care). The service may be purchased from the Switchboard at the time of admission or between 9:00 a.m. to 8:00 p.m. each day from switchboard.

Financial Services

Financial Services is in the area opposite Tim Hortons in the main lobby of The Brantford General Hospital. It is open Monday through Friday  from 9:00 a.m. to 3:00 p.m. All payments may be made by cash, cheques, credit card or debit card. The Switchboard will accept payments after hours.

Parking

There are a number of parking lots situated in convenient locations around the Hospital. The charge is $6 and exact change is required. Patients and visitors can obtain change from the Switchboard, the Financial Service cashier opposite Tim Horton's or at Gifts Plus in the lobby.

When picking up day surgery or in-patients, family members are encouraged to park in Lot C. Nursing staff will provide a parking pass to the patient. We ask that you abide by the by-law and not park in the fire route located outside the main doors.

Family members assisting in care may wish to obtain a temporary parking card from the Financial Services Business Office opposite Tim Horton's.

Clergy

Patients are asked at the time of admission whether they would like their religion noted and if so, the information is collected at that time.

Every effort will be made to assist you with your own spiritual needs. If you complete a request card or speak to the nurse, either the Hospital Chaplain or other clergy or spiritual advisor will be notified.

The Chapel is located on the second floor at the east end of the hospital and is open everyday for prayer and meditation and at other times on request. If you wish to attend Chapel service or use the Chapel for family or a small group, please speak to the nurse in charge of your area and arrangements will be made.

Pre-admission

To facilitate the admission process, patients are requested to bring with them a blue hospital card, OHIP card and any additional health insurance information they may need. Also bring a list of medications that you are currently taking.

Each time you visit The Brantford General or The Willett, Paris, you will be asked to confirm the information previously collected. Your Ontario Health Card will be verified for each visit, even if you have multiple visits on the same day.

In addition to the information above, you will be asked questions such as:

  • What is your date of birth?
  • What is your address?
  • Who is your next-of-kin?
  • Person to notify (if different from next-of-kin)
  • Who is your family doctor?
  • What is your Mother's maiden name?
  • What is your maiden name/Do you have any other name?

Dependent upon the procedure you will be undergoing, you may also be asked information about your current medical and physical condition. You may be asked to provide information regarding your personal and or family medical history.
